作者简介

王鲁，男，1968年10月生，主任医师、教授、博导，复旦大学附属肿瘤医院肝脏外科主任，国际腹腔镜肝脏外科学会（ILLS）创始会员，国际肝胆胰协会（IHPBA）会员，世界外科协会（ISS）会员，国际肝胆胰协会中国分会肝胆胰肿瘤诊疗协作组组长，国际肝胆胰协会中国分会肝血管瘤专委会副主任委员。兼任中国抗癌协会胆道胂瘤专委会副主任委员、中国医师协会结直肠癌肝转移专委会副主任委员、中国抗癌协会肿瘤转移专委会青委会副主任委员、上海市抗癌协会肝胆肿瘤综合治疗专委会主任委员、上海市中西医结合学会肿瘤微创专委会副主任委员、上海市抗癌协会肿瘤微创治疗专业委员会腔镜外科学组副组长等。先后主持五项国家自然基金（负责人），主持国家科技重大专项目“肝癌精准肝切除技术和微创化治疗精准技术研究”。参加国家重点基础研究规划项目（“973”）、国家“863” 项目、十五科技攻关项目，获上海市科技进步二等奖和中华医学科技二等奖各一项。以第一作者或通讯作者发表SCI论文30篇左右，单篇最高IF=11.71，总影响因子150分左右。

前　　言
转移性肝癌是其他部位原发的恶性肿瘤转移至肝脏，并在肝脏形成单个或多个的癌灶。肝脏是大多数恶性肿瘤较常转移的器官，恶性肿瘤致死病例中约40%的患者伴发肝转移，导致转移性肝癌发病率已远远超过原发性肝癌。目前国内转移性肝癌原发灶最多的器官是结直肠，其次为乳腺、卵巢、胰腺、神经内分泌器官、肺、胃、鼻咽等。
转移性肝癌的治疗，包括系统性化疗、免疫靶向治疗以及针对肝脏病灶的局部治疗，如手术治疗、射频消融、放射治疗等。外科切除在常见转移性肝癌中的治疗价值已得到越来越多研究的证实。截至目前，根治性手术切除已成为结直肠癌肝转移和神经内分泌瘤肝转移的最有效治疗途径。随着外科手术技术的进步如微创外科开展、加速康复外科普及，以及有效的全身治疗药物不断出现、个体化精准分子诊断的发展，转移性肝癌的外科治疗经历了理念不断更新和适应证逐渐扩大的过程。对于不可切除肠癌肝转移与神经内分泌肿瘤肝转移患者，肝移植的安全性和有效性也得到了初步证实。但是，当前满足手术切除指征的患者占比较少，术后复发风险高等问题也不容忽视，转移性肝癌的整体预后仍较差。未来，转移性肝癌的外科治疗模式将进一步优化，通过受益人群的早期识别、手术时机的准确选择，以及MDT模式的全程管理，将进一步向精准医疗和个体化治疗迈进。
在此背景下，编纂此书的想法提上了议事日程，我们组织了相关的专家与学者，在大家的共同努力下，经过一年多的反复修改与撰写，本书得以顺利编写完成。
