近年来,骨外科学发生了深刻的变化,已经从大骨科向脊柱外科、关节外科、创伤骨科、足踝外科、运动医学、小儿骨科、手外科等专科细化发展。足踝外科是骨外科学重要的分支学科之一,在我国却是一个比较新兴的专业。足踝疾病的治疗已经有很长的历史,近年来逐渐得到人们的广泛重视,开始进入了发展快车道,从事足踝外科专业的骨科医师也在逐年增加。随着足踝外科新理念、新技术、新方法层出不穷,同时人们对治疗效果的期望值逐渐增加,对足踝外科专科医师提出了更高的要求。作为足踝外科的难点与重点,足踝部畸形矫正对于医者充满挑战。鉴于此,我们编写《足踝矫形手术》,希望能为广大足踝外科医师的临床工作提供借鉴与帮助。在编写过程中,我们系统查阅了国内外相关资料,并对多年来西安红会医院足踝诊疗中心的病例和影像资料进行整理。全书共10 章,分别介绍了矫形术前查体与影像学评估、足踝外科矫形原则、足踝畸形( 外翻、平足、高弓马蹄内翻足)矫正、中后足病损及创伤后遗症矫正,以及康复的相关原则及内容。足踝部查体及评估对于制订手术计划具有重要意义,故本书设立独立的一章重点介绍足踝部查体与影像学评估,包括视诊、功能活动评估、影像学检查评估(X 线、CT、MRI、B 超)。尽管矫形需要个体化评估与治疗,但一些原则仍然需要遵循,故第二章重点介绍足踝部的矫形原则,包括基本原则、评估原则及手术矫形原则。第三章至第九章为本书主要内容,详细介绍了足踝外科领域每一种畸形常见的手术技术。在介绍每一种手术技术操作时包含引言、手术指征、手术禁忌证、手术技术、技术要点、术后处理和小结,重点突出手术操作技术及技巧。足踝部三大畸形( 外翻、平足、高弓马蹄内翻足)独立成章,详细介绍了临床常用的矫形技术、方法技巧及经验。足踝部退变及中后足病损也独立成章,重点对关节置换、关节融合以及多种保关节截骨矫形术进行详细阐述,其中丰富全面的病例可以给各个阶段的足踝外科医师深刻启发。第十章介绍了足踝部矫形术后康复训练的基本原则及内容。足踝部畸形病因复杂,个体化特征明显,各位读者阅读本书时,需前后参照,以评估为基础,以原则为导向,结合手术技术,方能制定合理的手术方案,达到的治疗效果。本书在文字方面一改传统的成段论述,采用了提纲式的撰写方法,尽量以短的篇幅给予读者更多的“干货”。这样的撰写方法逻辑层次清楚、简明扼要,也增加了本书的可读性。本书版式新颖,包括大量精美的原创示意图和手术实例图片,其中涵盖了很多我们临床中遇到的病例,图文并茂地介绍了各种手术方式的步骤,向读者阐明怎样去做手术,重点介绍了具有代表性和近年来开始实施的新术式。在每种手术操作介绍结束后,详细说明该手术技术要点和小结,为笔者及团队多年临床经验的高度浓缩,是本书的精华所在。西安红会医院足踝外科诊疗中心是我国较早设立的以诊治足踝部疾病和下肢创伤为主的专科,是中华医学会“全国足踝外科医师培训基地”,年门诊量达5 万余人次,手术量年均3 000 余台,目前承担、省市级科研项目30 余项,出版、参编及翻译专著10 余本。本书的编写团队涵盖了西安红会医院足踝外科诊疗中心多名具有丰富临床经验的教授、副教授。在本书编写过程中,我们尽量做到紧跟国内外手术的进展,并努力做到准确翔实,但足踝外科理念、器械发展日新月异,加之编写时间仓促,编者水平有限,难免有缺点和不足。诚恳希望专家与同道把意见、建议反馈给我们,以便得到及时更正与完善。梁晓军2022 年6 月
