《医患交流·癌症防治与康复系列丛书:甲状腺肿瘤百问百答》:
  胸部及骨骼X线片:常规胸片观察有无转移,必要时行骨骼摄片。本病可发生骨转移,以颅骨较多;次为胸骨柄、锁骨、肋骨、脊椎、盆骨、肱骨及股骨等。骨转移一般表现为溶骨性破坏,无骨膜反应,可侵犯邻近软组织。
  (2)CT诊断。CT检查对大多数病例可提出良恶性诊断依据,而且可明确显示病变范围,尤其对胸内扩展的病变范围以及与邻近大血管的关系.为制订治疗方案提供可靠依据。通过CT检查可确定肿瘤为单一还是多发,瘤体为实质性还是混合性,肿瘤内是否存在囊变、坏死、钙化及其形态,对囊性变者可通过增强扫描确定其形态,囊壁厚薄是否均匀,有无瘤结节,并且进一步了解甲状腺肿瘤对邻近肌肉组织、气管、食管壁、颈血管鞘的侵犯情况和是否存在颈淋巴结转移。然而,CT检查仍有其局限性,不可避免地存在误诊或漏诊,因此,在诊断时应注意与腺瘤及结节性甲状腺肿的鉴别。同时可结合超声、MRI、PET-CT及放射性核素检查,有助于提高乳头状甲状腺微小癌的早期诊断率。
  (3)磁共振成像。磁共振成像(MRI)可明确显示甲状腺肿瘤的范围及其与邻近组织关系。甲状腺癌MRI表现特征:①肿瘤非均质性改变,表现为肿瘤内信号不均匀,其发生机制与瘤内出血、坏死,囊变有关。当肿瘤生长速度超过微血管的生长速度时,肿瘤细胞就会坏死,同时也会出现囊变,也易侵犯周围血管和瘤体内部的血管而导致出血。而甲状腺良性病变也易发生出血、囊变等,因此肿瘤信号的不均匀只是恶性肿瘤的一个征象,单一的不均匀性信号不能可靠地鉴别肿瘤的良恶性,需要结合其他征象综合考虑。②肿瘤表现为不规则、分叶状。③瘤周不完整包膜样低信号影,这是MRI诊断甲状腺癌的特征性表现之一。包膜样低信号位于甲状腺肿瘤与正常组织之间.主要由包膜/假包膜(主要是纤维组织)构成,于TIWI和T2WI上均表现为低信号影。
  (4)正电子发射计算机断层扫描。正电子发射计算机断层扫描(PET-CT)是目前的、准确性较高的核医学显像技术,它能较早期地揭示生物机体的异常功能、代谢变化,甚至可在机体出现临床症状、体征或病变解剖形态发生改变之前发现病灶,从而有助于疾病尤其是恶性肿瘤病变的早期诊断、早期治疗及对疾病的发生、发展和转归的及时判断,而多排螺旋CT则可以显示人体精密的解剖结构,解决了PET显像所存在的解剖定位关系不清的缺陷,因此PET-CT将功能图像和解剖图像进行了精确地融合,能方便地检测出更细小的病变组织,并能提供精确的解剖定位,进一步提高了对肿瘤诊断的正确性。PET-CT的优势主要体现在:①确诊原发灶后,能一次全面了解潜在区域淋巴结及远处转移灶并完善分期;②术后评估残留病灶或复发;③部分患者通过其他常规影像学检查难以确诊,而又不愿接受细针抽吸等有创检查;④部分甲状腺良恶性肿瘤原发灶的鉴别;⑤PETCT诊断灵敏度较高,可为外科精确制订治疗方案提供有益参考。由于PETCT价格较高,无法作为常规检查,但不失为一个有益的补充手段。
