第1章 洗手这回事

 

12 月的某一天，我和传染病专家黛博拉 · 横江、微生物学家苏珊 · 马里诺一起在医院巡视。她俩在医院的感染防控组工作，专职负责就是杜绝医院内发生病菌感染和蔓延。这可不是个在人前闪耀光辉的工作，她们也都是非常朴实低调的人。横江 45 岁，说话低声细语，一笑就露出酒窝，上班老是穿着运动鞋。马里诺 50 多岁，天生内向，不爱讲话。尽管外表普普通通，但她们已经成功处理过多次危机，比如流感、军团病和致命的细菌性脑膜炎，而且就在几个月前，还控制了疑似疯牛病的传播。当时一位病人的脑部检查结果显示，他可能患了这种病，倘若传播出去，将会是一场噩梦，因为这种病不但无法治愈、会致人死命，而且其致病病毒无法通过常规的高温消毒手段杀灭。也就是说，在检验结果出来之前，神经外科医生使用过的器械可能已经将疾病传染给了其他病人，幸好感染防控小组的人员及时追查到那些器械的去向，并实施了化学灭菌。此外，横江和马里诺还亲历过麻疹、瘟疫和兔热病的防控战役。还有一次，她们发现一个聚会提供的一批草莓冰激凌引起了甲型肝炎的暴发，因而呼吁从全国范围内召回该品牌的草莓冰激凌。她们告诉我，近在医院里肆虐的有轮状病毒、诺瓦克病毒、几种假单胞菌、具有超强耐药性的克雷伯氏菌，还有现代医院里无处不在的祸根：耐药性金黄色酿脓葡萄球菌和粪肠球菌（以上几种均为传染性病菌，它们经常引发肺炎、伤口感染和血流感染）。

根据美国疾病预防控制中心的数据，每年有 200 万美国人在医院里受到感染，其中更有 9 万人死于感染。横江说，在感染防控组的工作中，困难的部分其实不是与各种各样的传染病对抗，也不是处理病患和员工中有时会产生的恐慌情绪，而是敦促像我这样的临床医生按照要求去做一件事，这件事做好了，就能够有效遏制感染扩散，那就是洗手。

她们采取的措施可以说是细致入微。她们在外科楼层里到处张贴提示海报；将原有的洗手池重新摆放位置，加装了一些新的，并将其中一些改造成自动式；专门斥资 5 000 美元购买了“预防手推车”，车中存放了一些方便携带、外观也很漂亮的小包，每个小包里洗涤剂、手套、手术衣等用品一应俱全；对执行效果好的科室，还发放免费的电影票以示奖励。尽管如此，情况依然没有得到任何改善。统计数据表明，我们医院的医生和护士洗手的次数仅为应有的 1/3 到一半，跟其他医院并无区别。我们跟流鼻涕的病人握手，从伤口处剥除粘住的衣物，用听诊器紧贴汗湿的胸口，之后大部分人顶多是在白大褂上蹭几下手，然后继续该干什么干什么：看下一个病人，在病历上狂写，甚至直接抓取食物。
