前言
早在三百多年之前,日本就已经能够制作出可以戴在口内并行使口腔功能的全口义齿,现在依然可以见到许多保留下来的木质义齿。然而这些木质义齿,在当时并不是随时随地、谁都可以制作出来的。从欧洲文艺复兴时期起,基于科学思考方式的近代口腔医学迅速发展。1920年,Gysi在前人研究的基础上,将全口义齿理论及技术进行整理,总结出一套全口义齿修复的方法。随着科学技术的发展、口腔材料的更新及医学教育的普及,如今,在现有的医疗制度下,在任何地方,患者无论去哪家口腔医院,基本都可以得到一副坚固耐用的全口义齿。制造业及服务业的宗旨是"为了他人"。现代,作为"他人"的"顾客"的满意度,包括安全感与安心感,已经成为评价工作成功与否的标准。口腔医疗领域也从追求"结实耐用的全口义齿"时代转变成不得不提供"患者满意度高的全口义齿"时代。那么,如何才能制作出患者满意度高的全口义齿呢--很多相关的优秀的书籍已经进行了阐述。在本书第二章,经验不甚丰富的平冈秀树医生,通过临床的观察、思考、学习以及5年以上的实践,对2500张照片精心筛选,以图文并茂的形式,详细阐述了全口义齿治疗的过程,包括从初诊的临床操作到与加工所之间的沟通等内容。因此,通过本书的学习,即使是全口义齿临床经验较少的口腔医生或技师,也能从本质上发现问题并将其解决。十九世纪初,有学者发现,在大气压力的作用下,全口义齿可以与黏膜面贴合,将义齿从口内取出时能感受到吸附力。全口义齿修复时应以自然现象为原点,以提升患者的舒适感以及符合患者生理规律为目的,追求佩戴时无异物感的全口义齿。即当询问患者佩戴全口义齿时感觉"如何"时,患者的回答通常为"没有不适感"。在日常生活中,患者咀嚼时无疼痛,大笑、聊天时没有任何异常,感觉义齿"大小刚好"、"松紧合适"、"高低正好"--即使患者有意识地察觉义齿有无异样,还是感觉"没有不适感",这是因为义齿与患者的生理功能非常协调。本书中所说的"没有不适感",体现了全口治疗与技术的本质,即获得患者或者说是
